This position is for a Registered Nurse to provide skilled home health care to patients in the South Bay/Long Beach, CA area and surrounding communities. The role involves direct patient care, charting in EPIC/OASIS, and adhering to a 1:1 patient ratio (up to 6 patients/day). Prior home health experience and specific certifications are required. This is a day shift position with a weekend requirement of one weekend shift per contract.

Responsibilities:
- Provide direct skilled nursing care to patients in their homes.
- Develop and implement individualized care plans.
- Administer medications and treatments as ordered by physicians.
- Chart accurately and thoroughly in EPIC/OASIS.
- Maintain accurate patient records and documentation.
- Communicate effectively with patients, families, and other healthcare professionals.
- Meet daily productivity standards (30 productivity points).
- Manage a caseload of up to 6 patients per day.
- Provide own transportation to patient homes.
- Occasionally travel up to 50+ miles for client visits (clients are typically grouped geographically).
Required Skills & Certifications:
- Current Registered Nurse (RN) license.
- Minimum 1 year of skilled home health nursing experience.
- BCLS certification.
- Phlebotomy/IV certification.
- OASIS experience.
- COVID-19 primary series plus one booster.
- Ability to provide own transportation.

Special Considerations:
- Must clearly state if lacking OASIS experience.
- Up to one shift per pay period may be called off.
- RTO (Requested Time Off) must be confirmed with the hiring manager.
- No call, float, or block scheduling.
- First-time travelers are not accepted.
- Local candidates (within 50-mile radius) are preferred, but accepted at the same rate.
- Pending license is acceptable.
Scheduling:
8-hour day shifts; one weekend shift per contract is required. No 4x10s or 3x12s are available.
