We are seeking a compassionate and experienced Patient Care Manager (RN) to join our dedicated home health team in Norwood, PA . This leadership role is essential in ensuring the highest standards of patient-centered care while fostering excellence across our nursing staff. Oversee and manage patient care services, ensuring compliance with state, federal, and Medicare regulations.
Lead, mentor, and support nursing staff to promote professional growth and improved patient outcomes.
Collaborate with interdisciplinary teams to design and implement individualized care plans.
Ensure accurate and timely clinical documentation and reporting.
Conduct audits of patient records and care plans to maintain quality assurance.
Serve as a liaison between patients, families, and healthcare providers to ensure seamless care coordination.
Drive quality improvement initiatives and participate in ongoing professional development.
Current Registered Nurse (RN) licensure in the state of Pennsylvania (required).
Minimum of 2+ years of supervisory or management experience in a home care or home health setting (preferred).
Strong knowledge of professional nursing standards, Medicare Conditions of Participation, and regulatory compliance.
Excellent leadership, communication, and organizational skills.
