Job Description

We are seeking a dedicated and experienced Nurse Practitioner or Physician Assistant to join our Family Medicine team as a floating provider. This role will support patient care across multiple outpatient family medicine sites in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties in South Carolina. The ideal candidate will be passionate about patient-centered care, preventive health, and chronic disease management, while building strong, trusting relationships with patients and their families.

Key Responsibilities:

Provide comprehensive primary care services to patients across the lifespan.

Travel regularly to assigned family medicine sites within the designated counties: Abbeville, Greenwood, Laurens, McCormick, and Saluda, SC

Deliver high-quality, evidence-based care with an emphasis on wellness, prevention, and chronic condition management.

Collaborate effectively with physicians, nurses, and other team members to coordinate patient care.

Educate patients and families about treatment plans, medications, and health maintenance.

Document patient encounters accurately and timely in the electronic health record system.

Starting Salary: $100,000

Requirements:

Graduate of an accredited Advanced Practice Registered Nurse/Nurse Practitioner program or Physician Assistant program.

Minimum of 2 years of experience in a primary care or family medicine setting required.

Active and unrestricted South Carolina medical license.

Certified or eligible for certification in area of specialty (FNP, PA-C).
