Qualifications:

• Active NY State Medical License.
• Board Certified or Board Eligible.
• Active Medicaid number & NPI.
• Spanish-speaking preferred.

Key Responsibilities:

• Diagnose and treat acute and chronic conditions.
• Provide preventive care, immunizations, and health counseling.
• Review labs, imaging, and adjust treatment plans.
• Coordinate care and refer to specialists as needed.
• Maintain thorough documentation.
• Support patients with long-term management of conditions.
