Position Overview
We are currently seeking an Inpatient Pharmacist Night Lead to join a leading healthcare facility. The Pharmacist Night Lead is an essential member of the multidisciplinary care team, responsible for overseeing pharmaceutical services during the overnight shift. The role includes supporting medication safety, antimicrobial stewardship, optimized pharmaceutical dosing, and providing leadership to other pharmacists working during the night shift.
Key Responsibilities
Lead a team of pharmacists and support staff to ensure quality pharmaceutical services during overnight hours.
Serve as a resource for medication safety, antimicrobial stewardship, and optimized drug dosing.
Review and interpret orders and prescriptions to ensure validity and safety.
Perform clinical activities including order verification, pharmacokinetics, IV to PO, and therapeutic interchange.
Provide direction and mentorship to support staff, ensuring a safe and efficient work environment.
Assist with medication histories, monitor patient drug therapies, and provide drug information as needed.
Work Schedule
Shifts: Full-time night shift, with occasional weekend and holiday rotations.
Hours: Overnight shift to support inpatient pharmacy services during off-hours.

Minimum Qualifications
Doctor of Pharmacy (Pharm.D.) degree preferred, or Bachelor of Science degree from an ACPE-accredited School of Pharmacy required.
Licensed Pharmacist in the State of Vermont required.
ASHP-accredited Pharmacy Residency or equivalent experience preferred.
At least 2 years of clinical pharmacist experience in a health system preferred.
