Job Description

We are seeking compassionate and experienced Registered Nurses to join our team in the Burke County area. As a Hospice RN Case Manager, you will play a vital role in providing end-of-life care and support to patients and their families during a challenging time.

What You'll Do:

Conduct comprehensive assessments and develop individualized care plans.
Provide compassionate and supportive care to patients and their families in their homes.
Educate patients and families about their condition, treatment options, and coping strategies.
Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals.
Maintain accurate and timely documentation.
Advocate for patient rights and ensure quality of care.
What You'll Bring:

Associate's Degree in Nursing (BSN preferred).
Current and unrestricted RN license.
Current CPR certification.
Minimum 1-year experience in direct patient care (hospice, palliative care, home health, etc.).
Excellent communication, interpersonal, and documentation skills.
Compassionate and empathetic demeanor.
